Case Management

 

The Case Management department at Western Medical Center Anaheim (WMCA) is comprised of nurses and social workers that have an extensive background in many clinical areas. Our services can be especially helpful for patients and families who may feel overwhelmed.

 

Our staff communicate daily with doctors, nurses, therapists, dieticians, and all those involved with the patient to coordinate the plan of care.  Patients are then given information in a format that is easily understood. 

 

·         Case Managers are specially trained registered nurses who serve as a patient advocate providing information and support for patients and their families.  They help to coordinate medical care for patients.

 

·         Social Workers assist the family/patient in formulating a comprehensive, effective and timely discharge by integrating individual medical and psychosocial needs, practical living problems, and strengths and dynamics of the family interactions with resources in the environment as needed.     Discharge interventions, including but not limited to, placements of patients         to: Skilled Nursing Facilities, Shelters, Board and Cares, Financial resource counseling/referrals and Community agency resources.

 

WMCA’s Case Management staff works closely with others in the hospital and in the community to evaluate, plan, carry-out and monitor a wide array of health services for our patients which may include social and emotional support services. These services take advantage of the best the hospital and the community has to offer. Our goal is to meet the individual's on-going needs in the most cost effective and efficient way, while promoting the best possible outcome for patients, families and the community.

 

Services include:

·        Discharge Planning - Putting together a plan of care for patients who are leaving the hospital. We work with patients, their families, their doctors, and community-based resources to meet the patient's on-going health needs.  Case Management has a wide array of information on home care, hospice care, skilled nursing facilities, and acute rehabilitation facilities.

·        Advanced Directives Advance Health Care Directives is defined as a written instruction that relates to the provision of healthcare choices when the individual is incapacitated.  This can be a “living will”, natural death act declaration, a durable power of attorney for health care, a “Do Not Resuscitate” (DNR) request, or a specific health care treatment directive.  Our staff is here to assist patients about their options and rights to make their own decision, and we will provide support and assistance to individuals desiring Advance Directives. 

·        Utilization Review is an ongoing process of assessing the use of professional care, services, procedures and facilities, including the medical necessity of such use.  This process includes monitoring the appropriateness of admissions, level of care, use of ancillary services, the need for continued stay and review of discharged patients.  This process is applicable to all patients admitted regardless of financial class.

·        Insurance Questions – Our staff is here to answer any questions regarding insurance including financial assessment and referral services.  WMCA contracts with most major Health Maintenance Organizations (HMO) and most major Preferred Provider Organizations (PPO) for hospital services. WMCA also participates in government programs such as Medicare and MediCal.


Our Services and Hours of Operation:

The Department is staffed Monday through Friday, 7:00 am – 5:00 pm; and on call weekends for emergency needs.
