Alvarado Hospital Medical Center’s Diabetes Education Program offers a variety of educational opportunities for people with diabetes.The goal of this program is to provide each patient with information for the self-management of diabetes, including providing helpful guidelines for family members. Special services such as foot care and management during pregnancy are also available.
Individual Maintenance Plan Certified diabetes educators are available to discuss your personal maintenance plan. This private consultation may include topics such as:
diagnosis
blood glucose monitoring
insulin administration
meal planning
exercise
the use of supplies for self-monitoring
Based on this consultation, a personalized program is developed, or changes in your current maintenance program may be recommended. Creating an individualized plan typically involves several appointments with a Diabetes Educator. During the first appointment, your current management program is assessed, and goals are discussed. Once a maintenance plan is defined, a three-month follow-up appointment and yearly assessments are recommended.
